Memorial Hermann Patients in Houston - Continuity of Care Forms

Putting out a little #journorequest: Would love to talk to anyone who's filed or is planning to fill out a "continuity of care" form following the recent split between Memorial Hermann Health System and Blue Cross and Blue Shield of Texas as reported this week by the Houston Business Journal's partners at KHOU 11. If you're planning to apply or know someone who is, send them my way [email redacted]! These forms are intended for patients who are currently receiving care at providers who are now out of network, and if the insurer OKs the form, those patients can keep receiving care at the network level.
